Richmond Texas, A worker assigned to perform routine Maintenance in an above ground water tank drowned in about 30ffw Wed. Oct. 13,2010 .

Rescue units from the sheriff's office, Richmond fire department, and Fort Bend county EMS responded to the scene.

An early investigation indicates the worker's tether line became loose soon after entering the water. When a second worker jusmped in to help the victim, He found the man unconsicious and unresponsive.

Authorites say the victim removed his mask for an unknown. reason As the second diver was trying to a****s the situation, he started having regulator problems and had to return to the surface.

As part of the rescue operation, firefighters dropped extra oxygen supplies down into the water tank??

The victim was removed from the water and declared dead a short time later.

The maintenance work, which involves removing silt from the bottom of the tanks, is performed once every two years.

The victim's identity has not been released, but authorities say the man is 28 years old and from Arlington, Texas. At this time, his death is being called and accidental drowning.

said he had pulled his mask off ( just looked on company web site and show's picture of KM EXO dive mask on diver also picture of diver in SCUBA ) News clip did not show any dive hose on top of tank.
clip also did not show any confined space entry equipment on tank top.
this is b*******. when are these mom and pop and small companies gonna get their s*** in line and make it safe?
Blame the ADC and their CFR's the "REQUIRED TRAINING" they demand has the rookie believing they are divers, and the CFR's themselves violate OSHA standard. where were the comms and the JSA. A proper JSA could have prevented this.
Must have been new to the business, a DIVER with experience would not go into a confined space without comm and a bailout. OSHA wll have a field day with thisone. The ADC needs t remove the word DIVER from their infamous Tender cards, this will at least dent the injuries to rookies who believe they are qualified divers when they graduate.
